No online forms. You cannot send us messages, appointment requests, or health information through this website. To reach us, please call.
We do not sell or share information about visitors to this website.

Patient forms

The patient forms on our Forms page are PDF files you download, fill out on your own device or by hand, and bring to your appointment. Nothing you type into them is sent to us or saved by this website.

Our web host

This website is hosted by Cloudflare (Cloudflare Pages). Like most web hosts, Cloudflare may automatically process technical information about each visit, such as your IP address, browser type, and the pages requested, to deliver the website and protect it against attacks and abuse. We do not use this information to identify visitors. See Cloudflare’s privacy policy.

Google Maps and Facebook

Our Contact page shows a map from Google Maps. When that page loads, your browser connects to Google, and Google may collect information under its own policies. Links to Google Maps, our Google reviews, and our Facebook page take you to those companies’ websites. They are separate companies with their own privacy policies:
